I authorize PACD to investigate all facts contained in my application for employment with said company and authorize the release of any and all information by my present and past employers, wherever located, which may be required for a reference check. I further authorize all of my previous employers and current employer to give any and all information concerning my employment and any other pertinent information which said employers may have, personal or otherwise, and I release all parties from all liabilities for any damages which may result from the furnishing of said information.

I agree to read and understand the PACD Substance Abuse Policy and agree to be bound thereby for purposes of applying for, and, if offered, accepting employment at PACD.

Specifically, I understand and agree to undergo substance (drug & alcohol) screening of my blood, urine, breath, saliva, or otherwise for purposes of assuming employment. I further understand and agree that once employed, upon reasonable suspicion I will be subject to further substance screening or face disciplinary consequences, up to and including loss of employment. I hereby authorize any PACD employee, designated physician, laboratory, hospital, or medical professional to conduct such screening and provide the results thereof to the company, and I release any such designated institution or person from liability thereof.

A copy of this release shall be as valid as the original.

To be considered for employment with PACD, you must meet the following criteria:

• Applicants must be at least 18 years of age and be a U.S. citizen or have the authorization to work in the U.S.
• Applicants must pass a background check showing no felony convictions.
• Applicants must have a valid California state Driver's License and a clean driving record.
• Applicants must possess a high school diploma or GED and have a minimum of 2 years of work reference.
